    How do I make a complaint or provide feedback?

    If you have a concern about your current NDIS supports or services, it is important that you talk about it. We value all complaints and feedbacks we receive. This helps us to improve our service and continue providing you with the highest quality of care you deserve.

    If you have any complaint or feedback of our service or staff member, you can submit a complaint to us or NDIS Quality and Safeguards Commission.

    All information we receive will be lodged with Centro Assist and will be treated with sensitivity, respect and privacy at all times.

    If you feel comfortable, we encourage you to raise your concerns with us first so we can seek to address the issue and provide you with a resolution as soon as possible. However, if you prefer, you can also contact the NDIS Quality and Safeguards Commission.

      What is NDIS Quality and Safeguard Commission?

      The NDIS Quality and Safeguards Commission (NDIS Commission) is an independent government body that works to improve the quality and safety of NDIS funded services and supports.

      How to contact NDIS Commission?

      A complaint can be made to the NDIS Commission by:

      Phoning: 1800 035 544 (free call from landlines) or TTY 133 677. Interpreters can be arranged.

      National Relay Service and ask for 1800 035 544.

      Completing a complaint contact form.

      What complaints can you raise with NDIS?

      The NDIS Commission can take complaints from anyone about:

      • NDIS services or supports that were not provided in a safe and respectful way
      • NDIS services and supports that were not delivered to an appropriate standard
      • How an NDIS provider has managed a complaint about services or supports provided to an NDIS participant
